About The Role:
Trident Home Health Care is seeking a compassionate and experienced RN to deliver skilled nursing services to clients in their homes. The RN will assess, plan, and coordinate care to promote optimal health and quality of life outcomes.
Responsibilities:

Conduct in-home assessments and develop care plans

Administer medications, treatments, and wound care

Monitor vital signs and clinical changes

Educate clients and families on health conditions

Collaborate with therapists, aides, and physicians
Requirements:

Valid RN license (Nursing Council Registration)
-
Minimum 1 year of home health or acute care experience
-
Valid CPR and First Aid certification

Strong communication and organizational skills

Experience working with CRM and EMR systems
